The 2011 Fred L. Soper Award for Excellence in Public Health Literature Nomination Form

Before completing this nomination form, please be sure that you have read and understand the award guidelines. Please ensure you complete all the required fields in your application.

Instructions: Please fill in all the requested information on the nomination form and attach a letter of nomination (two-page limit) signed by the nominator, a clearly readable copy of the nominated article, proof that the article was published in a scientific journal indexed in a medical database (e.g., PubMed, SciELO) or the Pan American Journal of Public Health in the past three years, and a curriculum vitae of the person accepting the award on behalf of the article (the first or corresponding author).

Nominations may be completed by any researcher, journal editor, or health professional including self-nomination. Unfortunately, we are unable to consider unsigned documents.
